Required Paperwork for Tryouts

PHYSICAL: All physicals must be dated after May 1, 2025 (PIAA Physical Form: PIAA Physical Form)

• Physical must be submitted before player is allowed to participate in tryouts

DRIVING/RIDING PERMISSION Form: Required by WCASD
